To Patients to be hospitalized
This hospital is responsible for acute care.
This hospital is an "acute care hospital" where many of the following people come; those transported by ambulance, those who need emergency hospitalization and emergency surgery. We want to accept as many emergency patients as possible and to provide emergency care. Therefore, please understand and cooperate with the following points.
- 1.Based on the inpatient treatment plan indicated by the doctor in charge, if the patient is making satisfactory progress, he/she will be discharged from the hospital by this time, and after discharge, he/she will recuperate at a medical institution referred by this hospital, located in the region or at home.
- 2.In order to give priority to the treatment of emergency patients and critically ill patients, we might request a sudden transfer of hospital rooms.
- 3.In principle, the discharge time is in the morning in order to accept patients needed to be hospitalized later.
The Rules for patients (Responsibility)
We believe that building trustworthy relationship between patients and medical professionals is the most important part of medical care. Therefore, while you are in the hospital, we would like to request you to cooperate and follow medical instructions in order to get curative effects. In case there is any disorderly conduct in the hospital, we will decline medical treatment.
- 1.Acts that significantly oppose to the rules for recuperation such as drinking, smoking in wards or hospital premises
- 2.Acts that significantly trouble other patients such as intentional shouting or causing a disturbance etc.
- 3.Acts that significantly hinder medical treatment such as use of foul language, violence, sexual harassment, etc.
- 4.Acts that are significantly uncooperative towards medical treatment such as refusal of treatment and examination without any reason.
- 5.Criminal acts such as assault, bodily injury or theft in the hospital
This is a DPC-target hospital.
As a medical institution that meets certain standards, we have introduced the DPC/PDPS (payment systems based on Diagnosis group Comprehensive Evaluation by Classification), a method for calculating hospitalization costs that is applied to insurance medical institutions approved by the Ministry of Health, Labour and Welfare. Therefore, it would be appreciated for you understand and cooperate with the following points.

Unlike the conventional "volume payment method" calculated for each medical treatment, it is an accounting method that calculates medical expenses according to type of disease and symptoms of inpatients based on the "diagnostic group classification score per day" established by the Ministry of Health, Labour and Welfare
You cannot visit another medical department
If after being hospitalized, you wish to have treatment or examination that is not directly related to the disease for your hospitalization, you are requested to visit your outpatient clinic or a nearby hospital after discharge. However, this is not the case if treatment is urgently needed.
You cannot visit other medical institutions
After being hospitalized, in principle you cannot receive medical examinations at other medical institutions (hospitals, clinics, etc.) or receive medication prescriptions. (The family members of the patient cannot receive medicine from a medical institution on behalf of the inpatient either). In case you wish to visit another medical institution while being hospitalized, please be sure to contact the doctor or the nurse in charge.
Please prepare the following before hospitalization
Belongings on the day of hospitalization

- bathing supplies & shampoo (※1)
- chopsticks, spoons (※1)
- cups (※1)
- tissue (※1)
- Underwear
- footwear (non-slippery)
- medicines currently taken
- medicine pocketbooks, Medicine information referral documents
- patient registration ticket, health insurance cards, long-term care insurance cards, limit application certificates (those who have them)
- A set of document files during the hospital stay
※In case you make a request for “the hospital-stay set” (marked ※1), the items (marked ※1) can be provided at the time of hospitalization.
※The items provided vary depending on the content of the request.
※Please label your property with your name.
※Daily necessities and medical care supplies necessary for hospitalization are available at in-hospital convenience stores.
Please bring the medicine you are currently taking
Please bring the medicine you are currently taking (including eye medicine, ointment, adhesive skin patch, insulin, etc.) and "medicine notebook" and "medicine instructions" in a medicine bag designated by our hospital.
Procedure on the day of hospitalization

Please come to the hospital at the scheduled time.

Please leave the medicine (pink bag) you brought at the medicine delivery counter on the 1st floor in the North Building.
※Please bring the only internal medicine necessary for the day of hospitalization (small bag) to the ward. (Do not leave it at the counter)

Please come to the admission desk on the 2nd floor in the North Building.

After the procedure, the staff will guide you to the ward.
※In case you are hospitalized on a holiday, please come to the emergency desk on the 1st floor in the Middle Building.

※Please refrain from visiting in the morning in order to avoid hindering medical care.
※Please do not eat in the patient’s room.
※Please refrain from sitting on a patient's bed or doing any activities that might contaminate inside the hospital.
※In principle, children under 12 years old (elementary school aged) are not allowed to visit. In case you have any special request, please contact the staff station.
- Children under the age of 12 might be infected by mumps, measles, colds, flu, etc.
- Inpatients are more likely to be infected than usual due to a decrease in physical strength and immunity level. Similarly, children can be infected by patients due to their insufficient immunity.
- In the case of children running in the hospital, it might cause an accident by colliding with patients. Also, loud noises made by children may interfere with the medical treatment of patients.
※Wearing a mask is requested when flu is prevalent.
Contacting inpatient
In order to protect personal information, we do not respond to inquiries by telephone from outside, such as a patient’s room number or the patient’s absence/presence.
We do apologize for any inconvenience this may cause and appreciate your understanding and cooperation.
Use of mobile phones
We have restrictions on the use of mobile phones by categorizing the mobile phone usable areas in the hospital into the following three:

※Please refrain from videotaping and taking pictures inside the hospital in order to protect patients’ privacy.
※Regarding use of mobile phones, we would like to ask you to follow the rules and to use them in a restrained manner. Please note that we will restrict their use if you do not follow the rules, public manners or cause trouble to others.
